Otter Health
Today, Otter Health comes out of stealth and announces its $8M Seed round, co-led by Fika Ventures and Evidenced, with participation from Slow Ventures, Metrodora, Layout Ventures, Patient Capital, and Semper Virens. We're proud to be Otter’s first institutional investor.
In March 2025, we met Keith Florance and Dr. Amol Navathe. They had already built and sold a company together, Embedded Healthcare, to Clarify Health, and were starting their second act. Keith came up on the payer side of healthcare at Evolent, BCG, and Clarify. Amol is a practicing physician and Professor of Medicine and Health Policy at Penn. They recruited Ram Keralapura, formerly Head of Data Science and ML at Google Cloud Enterprise Security, as CTO.
A founding team with a prior exit together and a shared, contrarian view of a massive problem is a combination we rarely see in healthcare.
Our Thesis
CoFound backs founders who redesign complex industries from first principles. Keith and Amol have spent their careers on both sides of the payer-provider table, and that vantage point lets them rebuild the model rather than automate the status quo.
Healthcare's largest costs sit in the friction between payers and providers, and every dollar of that friction is administrative spend with no clinical value. Otter Health is building the next generation of healthcare infrastructure by connecting provider-led utilization management with prior authorization relief.
A Paradigm Shift
Prior authorization is healthcare’s most hated process. Physicians complete nearly 40 a week, 93% say it delays care, and over 60% of low-value spending bypasses it entirely. Every well-funded competitor attacks this with workflow automation: faster submissions, faster reviews, fewer faxes. That speeds up a process that doesn’t work. The friction stays, and most of the waste never gets caught.
Otter attacks the root cause through provider-led utilization management: a real-time standard-of-care AI engine that evaluates each clinical decision against evidence at the point of care, directly from the EHR. Providers who practice to the standard earn relief from prior auth. Decisions outside the standard get flagged with an alternative pathway. Payers gain visibility into the low-value care prior auth never touched.
The result is a real-time feedback loop between providers and payers. Incremental automation makes prior auth cheaper to run. Otter's approach will obviate prior auth altogether.
Early Results Are Undeniable
Otter is already live in production across several payer and provider organizations (to be announced soon). Where health plans have removed prior authorization, around 95% of clinical decisions stay concordant with the standard of care, and plans are capturing savings on low-value care that prior authorization never reached. That is the evidence the industry has lacked: a plan can remove the authorization barrier and keep granular oversight of care appropriateness. Rollout into additional plans, health systems, and specialties is underway.
